Each device is uniquely identified and labeled with the date and the time of application as well as the name of the person authorized and the reason behind the lockout/tag-out (repair maintenance, repair, etc.). It must also be clearly stated that these devices are not able to be removed until an entire LOTO procedure has been carried out.
